FRM - 40408 Error when Updating a Table with New CTX Indexes.

Symptoms
1. When a Form tries to update a table with new ctx indexes the following error occurs:
Error Message: FRM-40408: database commit failure
2. No error occurs when commits are accomplished from sqlplus.
3. No errors exist in the alert log related to the FRM-40408 error.
4. CTX indexes are valid.
Changes
Upgraded Database from 11.2.0.3 to 11.2.0.4. This previously worked in the 11.2.0.3 Database.
