Errors When Using a Custom Login Page with FORM For serverRequestCacheType In OAM (Doc ID 1669726.1)

With OAM 11R2PS2, when a Custom Login page is set and the Request Cache Type is set to Form, the Login process ends with error.

The OAM diagnostic log shows the following error:
