Login Page Not Being Returned on Load Balanced OAM

Symptoms
You have Integrated Oracle E-Business Suite Release 12 with Oracle Access Manager 11gR2 (11.1.2) using Oracle E-Business Suite AccessGate.
When you try to access Oracle E-Business Instance via URL https://myhost.mydomain it should redirect you to OAM login page of a load balanced OAM server(s) but this is not happening, although a single OAM instance is recognized.
In an HTTP headers trace you see:
