Symptoms

When accessing an OAM protected resource, user gets below error on the browser

Oracle Access Manager Operation Error

The Access Server has returned a fatal error with no detailed information.

Contact your website administrator to remedy this prob

OAM diagnostic logs shows below messages

 

However, there is an explicit policy defined under application domains for the requested resource (<PROTECTED_URL>/** in this case)
