Document Task Flow Shows a Broken Icon for .PSD Files

Symptoms
Using IE, the icon next to the name of file have an "X" sign and the name of the file on it. Like this: "X Open test.psd test psd"
Using FF, there is no icon next to the image, so no broken image, no image at all. Like this: Open test.psd test.psd
Using Chrome, It shows a file icon with unnecessary blank space before and after the icon.
