'Dsadm Upgrade' Command Fails

In certain situations, the command 'dsadm upgrade' may fail to complete. Such a failure will look like the following...
dsadm upgrade /ldap/slapd-example
Directory Server instance '/ldap/slapd-example' will be upgraded to version 220.127.116.11.0.
It will no longer be usable with DSEE 6 commands.
Verifying configuration file ...
