OID 11g Instance Fails to Start or Restarts Over and Over (Doc ID 1684307.1)

Symptoms

Unable to connect to OID server.  All commandline attempts fail with "Cannot connect to the LDAP server"

A review of the OID logs in Oracle_Instance/diagnostics/logs/OID/oid1, you may find the oidldapd process dies and restarts over and over (many oidldapd01sXXXX.logs created with different PID #s, indicating that OID is dying and restarting.  Also oidldapd_stack files generated.)

OID may work for a few moments once is first starts up but then begins to fail.

Found only the following messages in the oidldapd logs:
