Symptoms
Oracle Access Manager 11g (OAM) has been configured to protect an application.
A custom login page has been configured in the OAM authentication scheme, the login page is deployed on a different server than the OAM managed server i.e. it is "external".

OAM Server RequestCacheType has been set to FORM and the custom login page is sending the OAM_REQ value as a Post Data parameter when credentials are submitted.
When the protected resource is accessed the custom login page is displayed.
When valid credentials are submitted the OAM blue "System Error" page is displayed.
The OAM diagnostic log shows the following WARNING level error:
Steps to reproduce
1. Access the OAM-protected resource in the browser.
2. The custom login page is displayed.
3. Submit valid credentials.
4. The OAM blue "System Error" page is displayed.
