Forms/Reports Installation Shows Frmweb Processs Increasing, The FMWC Fails With "Unexpected Login Errors", and Opmnctl Status Fails After Database Crash (Doc ID 1913519.1)

Symptoms

Ater the database crashed and was restarted which was associated to Forms/Reports installation, the following problems starting happening with Forms/Reports:

 

When trying to login to FMWC, it received an "unexpected login error" as shown.

 

The "ps -ef | grep frmweb | wc -l" command shows that the frmweb processes are increasing in count.

The "opmnctl startall" and "opmnctl status" show the following errors:
