Symptoms
Using Oracle Forms 11gR2, when configured the OID 11.1.1.7 with multiple realms (e.g. DEV, GLD, BAS, TST) for the different environments.
Forms-based applications are throwing errors when authenticating users.

Other errors that may be seen are:

Changes
Configured the OID from single realms to multiple realms or a container other than cn=users.
