"Internal Server Error 500" when trying to access Task Forms after integrating SOA/BPM with OAM

Symptoms
In this situation tasks are deployed on a seperate managed server. When attempting to open instances that are using those tasks forms you get an "Internal Server Error". Checking the separate server that's hosting the ADF task forms you see an error:
