Unable to save URL, UCM or ECM type Attachments in a BPM Human Task Form after applying 11.1.1.7.4 or 5 (BP4 / BP5) (Doc ID 1944771.1)

Symptoms

After upgrading to 11.1.1.7.4 you are unable to add attachments.  Although it allows the selection, it does not actually retain them after saving.
The issue occurs for URL type attachments and "Associate Webcenter Content document" type attachments only when "All Process Participants" option is selected and does not occur when "Only task participants" is selected.
