OGG Monitor Agent 12.1.3 Fails To Register With Monitor Server 12.1.3 with OGGMON-20283 error

Why GoldenGate Monitor Agent 12.1.3 -> ogg_agent.log shows the following errors:

 JAgent is unable to establish a connection to Monitor Server. Monitor Server is probably down. Retry again in 10 seconds.
 JAgent connection info: Jagent Host=<FULLY QUALIFIED HOSTNAME>, Jagent JMX Port=5556, Jagent Config Dir=/app/oracle/oggm/agent2/cfg, Monitor Host=<FULLY QUALIFIED HOSTNAME>, Monitor Port=5502,
and targets are not getting registered in OGG Monitor Server 12.1.3?

Another variation of this error is that the ogg_agent.log show no error.  But the monitor.log show a timeout connecting back to the agent:
