OAAM Forgot Password Shows ATM PinPad Instead Of Expected Challenge

When using default OAAM forgot password functionality, a user clicks on forgot password but does not see the expected challenge KBA page. Rather, they encounter an ATM pin pad page. 

Examination of OAAM logs may reveal lines similar to the following:
