Smart Update Uninstall Patch Fails with Message "Patch not installed" (Doc ID 1970064.1)

A customer reports attempting to apply a patch and getting a conflict message: "Patch A is mutually exclusive and cannot coexist with patch(es): B". However, when trying to remove patch B, Smart Update fails with a message "Patch not installed: B". The jar file for Patch B is present in the BSU cache_dir directory. Customer reported that they typically take the patch-catalog_xxx.xml file included in a patch zip file and rename it to patch-catalog.xml.
