Symptoms

OAM 10.1.4.3 BP13 ISAPI WebGate, REMOTE_USER header not being set per authn/Authz rule.

Steps to reproduce:

1. Open new browser session
2. Call ASP page that dumps out header
3. Login displayed
4. Enter USERNAME and PWD
5. Resource is displayed, No REMOTE_USER set
