Process with multiple start events gets stuck at the start activity

After applying the 22.214.171.124.7 Bundle Patch, instances created in a multi-start process won't move passed the start activity. There might not be any noticable exception in the logs. Some customers have seen this exception, which is very vague:
Another symptom is that there will be a new row in the CUBE_INSTANCE table (which belongs to the SOAINFRA schema) for the given ECID, but there will not be rows in the WFTASK table (also in the SOAINFRA schema) for that same ECID.
