OHS 11g Upgrade to 12c Fails with "An invalid source directory was specified."

Upgrade Environment Requirements
- At least two nodes in use. (For the remainder of this note, node 1 and node 2 will be used to differentiate hosts)
- At least one OHS 11g is on a separate node from the Adminserver AND Associated to Domain.
Node 1: WLS 11g Domain with Adminserver
Node 2: Standalone OHS 11g associated to Domain on node 1
This is a common setup for Enterprise Deployments.
While upgrading OHS 11g to 12c, Upgrade Assistant (UA) fails upgrading the domain component configurations with:
Instance home: /Oracle/Middleware/Oracle_WT1/instances/instance1
Instance id: instance1
System component named ohs1 of type OHS IS a candidate for upgrade
Instance home: /Oracle/Middleware/Oracle_WT1/instances/instance2
Instance id: instance2
System component named ohs2 of type OHS IS a candidate for upgrade
System component 12 to 11 mappgings
ohs1 maps to ohs1 in /Oracle/Middleware/Oracle_WT1/instances/instance1
ohs2 maps to ohs2 in /Oracle/Middleware/Oracle_WT1/instances/instance2
Finished examining CAM.SYSCOMP with status: SUCCESS.

Cause: An invalid source directory was specified.
Action: Select a valid Oracle HTTP Server component configuration directory.
Finished examining OHS.CONFIGUPGRADE with status: FAILURE.
