Moving Oracle Access Manager 10g (OAM 10.1.4.3) To New Policy/User Store Resulted Error "There Was A Problem Obtaining The User ID...."

Login to Policy Manager and Access Server Console throws error below on the browser.
The Access Server and Identity Server are both on the same host.
Issue occurred after configuring existing OAM 10.1.4.3.0 to new Policy/User Store in Identity Server.
This was done following E12493-01 Appendix D: Changing Directory Server Hosts.
After Appendix D.5 "Re-configuring the Policy Manager", login to Access Server Console, getting above message.
