Exalogic Virtual: Guest vServers Not Accessible With "vhub_update_init received out of order update" Errors After IB Switch Replacement (Doc ID 2083417.1)

In Exalogic Virtual racks, issue of vServers not accessible after the IB Switch replacement is observed.

Below "vhub_update_init received out of order update" errors are seen in the xm console window of the guest vServer.

At the time of the issue we will not be able to reach to the login prompt using xen console. Interfaces will not come up.
