Symptoms
This problem occurs in Forms and Reports 11.1.2.2 which has embedded OHS 11.1.1.7.0.
OHS 11.1.1.7.0 embedded in Forms and Reports 11.1.2.2 crashes frequently (every 2 weeks on average).
The 'mod_wl_ohs' is being used.
This error happens in "ohs1.log" file.
Changes
Install Forms and Reports 11.1.2, then upgrade to 11.1.2.2.
