Multiple UI Issues with Warning "Cannot initialize data provider - java.lang.NullPointerException" in the Logs Following an Upgrade (Doc ID 2092933.1)

The following Warning is seen in the $DOMAIN_HOME/servers/OIMMANAGEDSERVER/logs/OIMMANAGEDSERVER-diagnostic.log and/or $DOMAIN_HOME/servers/OIMMANAGEDSERVER/logs/OIMMANAGEDSERVER.out file and may be linked to multiple errors or abnormal behaviors following an upgrade to OIM or above.
