Production WCC Server Stopped Responding For All Requests Even Though the Status Was "Running" in WLS

Upgraded from WCC 188.8.131.52.0 to WCC 184.108.40.206.0 (220.127.116.11.0PSU-2015-08-08 12:15:29Z-r132185 (Build:18.104.22.168)) and on WLS 10.3.6. A 4 node cluster setup that was using NodeManager to restart nodes.
Two UCM nodes were not responding for any requests and were not accessible through URL, but the server's status was showing "RUNNING" in WLS.
The below error could be found in Content_Server_Logs.out file:
