Track Requests Having Duplicate Request Id's (Doc ID 2104094.1)

Symptoms

When user A clicks the 'Track Requests' option and displays their requests, they are seeing each request twice in the list. There is another user B, that has the same display name as user A and this may be a factor in this issue.
