Receiving Pop-up Error 'The view <undefined> does not exist' When Creating a Database Provider (Doc ID 2109167.1)

Symptoms

When trying to create a database provider and selecting the database type, we get the following pop-up Error:

Steps to Reproduce:
1.  Log into Content Server
2.  Click on Administration
3.  Click on Providers
4.  Add a database provider
5.  Select database type
6.  Receive pop-up box that says "The view <undefined> does not exist."

You can not do anything until after you click through "OK" several times.
