Duplicate Revoke Entitlement Request Leaves Orphan Request

Information in this document applies to any platform.
1) One user submits a revoke entitlement request for a disconnected application instance
2) Another user with rights or the same user submit again another revoke entitlement request for the same entitlement
3) The approver will get two different requests in his Inbox to revoke the same entitlement
4) On approving one
the approver will be left with a revoke request for an entitlement that does not exist anymore
This will lead to an orphan request that will not let take any action. How to resolve this issue?
