APEX Using EPG and Custom Image Prefix Displays Blank Pages

Symptoms

APEX is installed and configured to use the Embedded PL/SQL Gateway (EPG) as its listener. Setting a non-default image prefix at the time of APEX installation, or resetting the image prefix using the reset_image_prefix.sql, results in blank pages when APEX is accessed using the EPG.

Customers using APEX 5.0.X may receive a message indicating:

"There is a problem with your environment because the Application Express files have not been loaded. Please verify that you have copied the images directory to your application server as instructed in the Installation Guide. In addition, please verify that your image prefix path is correct. Your current path is [custom image prefix] (it should contain both starting and ending forward slashes, such as the default /i/). Use the SQL script reset_image_prefix.sql if you need to change it."

Dismissing that message by hitting the OK button also leads to a blank page.

Changes

A non-default image prefix was chosen at the time of APEX installation, or was changed after APEX installation using the apex_epg_config.sql file.
