JVM Controller Fails to Start or Errors with PDE-JM001 in Forms 12c

Using Forms 12.2.1, you configure a JVM Controller by following steps in either:

However, the JVM controller fails to start either from EM (FMW Control) or command line.

Failure to Start from EM:
