Update for 12.2.1.3:

Below remains for history on previous version...

In Forms and Reports 11g R2, OHS can be configured together with Forms&Reports by selecting OHS component in the following configure screen:

A.2.11 Configure Components
https://docs.oracle.com/cd/E24269_01/doc.11120/e23960/install_screens.htm#FRINS215

In Oracle Forms and Reports 12cR2, each product configuration steps can be found from the following documents:

Oracle Fusion Middleware Installing Oracle Forms and Reports
https://docs.oracle.com/middleware/1221/formsandreports/install-fnr/install.htm#FRINS101

This document describes how to configure Oracle Forms and Reports 12cR2 and Oracle HTTP Server 12cR2 together in single domain. For a final production system you may also configure an Oracle HTTP Server in a standalone domain on a separate machine to distribute the topology.  You may then be using the collocated OHS only for FMW Control and a remote OHS for your application. See the following document for this case:

Oracle Fusion Middleware Installing and Configuring Oracle HTTP Server
