Tuxedo /Q Queue Corruption Followed By TMQFORWARD And TMQUEUE Processes Dying (Doc ID 2134136.1)

Symptoms

Seeing Tuxedo /Q Queue corruption with Tuxedo 12.1.3
TMQFORWARD and TMQUEUE processes "died" messages of this type in the ulog (when queue reaches message limit):

LIBTUX_CAT:1477: ERROR: .SysServerDied: TMQFORWARD, group MY_QSPC, id 1765 server died"

Processes will not restart until the Tuxedo /Q queue, or a Qspace which is insane, is re-initialized.

The TSAM Standalone Agent is enabled in this configuration. The LMS server is running(with a large policy definition).
