Oracle HTTP Server Start Failed With "Cannot load ... mod_onsint.so into server: The specified module could not be found" (Doc ID 2141737.1)

Symptoms

Oracle HTTP Server(OHS) 11.1.1.1.9 start fail with error

OHS starts successfully if commented out “Loadmodule … mod_onsint.so” and “include …admin.conf”.

Tried following without success:
-Replaced mod_onsint.so with copy from a working instance
-Start OHS manually without opmn
-Windows ProcessMonitor filtered on mod_oninst.so did not show any error when starting OHS. 

Tried following on another server and issue does not reproduce
-Used httpd.conf from problem instance
-Applied same patches as problem instance
