Tuxedo Application Server Keeps Returning XAER_RMFAIL, When Starting An XA Transaction, Until Server Restarted

Symptoms
Tuxedo 11.1.1.2.0 application server is connected to, and executing XA transactions with, IBM MQ.
After a failed transaction is rolled back the involved application server remains in an "unhealthy" state. From that moment on, every time the application server attempts an xa_start it returns “ -7” (XAER_RMFAIL).
This situation lasts until the application server is restarted. LIBTUX_CAT:481, and LIBTUX_CAT:1274, error messages are seen in the Tuxedo ulog.
