Using Forms 12.2.1, calling a form from a menu or a button generates: FRM-40350 Query caused no records to be retrieved

BEHAVIOR

The issue can be reproduced at will with the following steps:
- form that does a simple query from the database to populate some rows - works fine
- call the same form from a menu (eg with a button to invoke) - FRM-40350 generated

- another scenario: Using a form that contains a Pre-form trigger with a null and a When-New-Form-Instance trigger that contains a select statement that returns no records.

When the form runs and executes a query, error FRM-40350 is generated. Removing the triggers leads to the forms working ok.
