Blank Screen When Accessing Forms

A blank screen occurs when attemptingto access any Form from a browser using http://machine:port/forms/frmservlet
The problem also occurs with Internet Explorer and Firefox when calling the test form, test.fmx
Certified versions of the browser and JRE are being used for the Forms application.
