Frequent WCS# Internal Error when Navigating Across Subspaces (Doc ID 2189728.1)

While navigating through the application, the pages keep coming. On the UI, it complains about some 'Internal Error'. If a user click any link, then user get this error:

Internal Error (WCS#2016.
If the error persists, report the problem to your administrator.

Invalid index

In the logs you see a whole error stack with that same time-stamp and execution-id which indicates that there were a few things going on that culiminated into this error. 

Internally, it seems to start with an ADFC-5001x type of error for invalid index. Something like this (from webcenter diagnostic logs)
