Oracle Access Manager (OAM) Upgrade Fails on Patch Set Assistant (PSA) for Audit User

Oracle Access Manager 11gr2ps3 (OAM 184.108.40.206.0)
Upgrading Identity Access Management (IAM) stack from 220.127.116.11.9 to 18.104.22.168.
New Oracle Access Management Social and Mobile (OMSM) schema has been created and now running the PSA to upgrade the existing schema's.
After the OAM schema is selected (which then selects a few others), it fails when upgrading the IAU schema.
It seems that the IAU schema is already upgraded, the tables and columns that it is trying to create already exist. However PSA does not simply write it off as "already upgraded" but failed.
