Symptoms

OAG is clustered and all instances have to get the synchronized value in order to apply a throttling filter.
As such a distributed cache was created via Policy Studio with the default configuration.
But when checked in the OAG Manager console, the replication was failing. This case was intermittently happening. However, every instance in the cluster can connect to each other.
