Oracle HTTP Server Target Reported as Down in Grid Console when the Server is Actually Up and Running

Symptoms
OHS not showing 100% availability in EM12c console
OHS targets are being monitored in 12c Grid control.
Intermittently, the grid control reports these targets to be down even when its up and running as verified from the opmn output.
Changes
The following commands were run every minute to check the OHS status reported by opmnctl and wlst.sh.
The output of the above command shows "Status:0" multiple times when the OHS server is actually running , confirming that the dms is providing wrong status output
