"Failure of Web Server bridge" Error When Trying To Access Forms Application

Failure of Web Server bridge error seen while accessing Forms Application and browser shows below message:
No backend server available for connection: timed out after 10 seconds or idempotent set to OFF or method not idempotent.
The following or similar errors are seen in the OHS component log. It is unknown why the IP address 198.x.x.x is reported as this is not a valid IP Address
