Oracle Access Manager 11gr2ps3 (OAM 11.1.2.3.x) SAML Assertion Failures with Error "Invalid SAML assertion"

Trusted Header Assertion has been configured per below document

Trying to access protected resource produces below error with assertion failure

<Entering OAMIdentityAssertionProviderImpl.assertIdentity with [OAM_IDENTITY_ASSERTION] token>

<SAML Assertion not present in http request header>

<Invalid SAML assertion>
