Symptoms

Oracle Access Manager (11.1.2.0+) Resource Access Failes With an Attempt to Redirect to /pages/servererror.jsp

When attempting to access a WebGate protected resource, the access fails with a 404 page not found on the http(s)://<OAM host:port>/pages/servererror.jsp page.  Investigating using a header trace tool, we see that there is no redirection at all to the OAM managed server for the authentication:
