Cannot Register Provider in Portal when Provider name was used Before (Doc ID 226225.1)

Symptoms

Portal 9.0.2.0.1
Unable to register a Portal provider when the new provider has the same name as a de-registered provider.  It is not possible to access the new provider via the portlet repository and receive error:

Error: User-Defined Exception in the refresh log

No error is given when re-registering the provider.
