Unable to see Org UDF After Upgrade from OIM 11g R1 to OIM 11g R2 PS3 (Doc ID 2266320.1)

After upgrading from 11gR1PS1 (  to OIM R2PS3 ( The Org UDF is not showing up under Sysadmin --> Upgrade --> Organization

The issue can be reproduced at will with the following steps:
1. Login into System Admin
2. Click on the upgrade Organization Form
