The "Request-Response" Correlation Pattern Is Not Working in BPEL (Doc ID 2268040.1)

Symptoms

Using "request-response" correlation pattern, results in correlationViolation error.

EM console fault:

<bpelFault><faultType>0</faultType><correlationViolation xmlns="http://docs.oasis-open.org/wsbpel/2.0/process/executable"></correlationViolation></bpelFault>

In log there is no exception, but it shows the following message:

** Scope Id: BpSeq0.3
** Audit Event Date: Fri, 17 Jun 2016 14:53:40.674 CDT
** Audit Message: Correlation set "Correlation_Set" is not initialized. It
cannot be used in a receiving activity. Fault is thrown.
** Audit Detail:
