Cannot Login To Forms Application - "Your security settings have blocked a signed application from running" with JRE Security Set to Very High (Doc ID 2277326.1)

Symptoms

Running forms and getting java security warning, "your security settings have blocked a signed application from running" is displayed in the java console:

com.sun.deploy.security.BlockedException: Your security settings have blocked a signed application from running
at com.sun.deploy.security.BlockedDialog.show(Unknown Source)
at com.sun.deploy.security.TrustDecider.validateChain(Unknown Source)

FRM-92091 unexpected fatal error in client-side java code displayed to user.

The JRE will display a blocking message similar to the following:

• Customers may also see this behavior if they were previously using the JRE Exception Site List with the Forms application and the URL has been removed from the Site List. 
• The security setting for the JRE is set to Very High.

Changes

 Java upgraded on client machines and the most current Forms jar (patch 19933795 for 11.1.2.2) files are present on the Forms server.
