Using the "All Application Instances" Option on Certification Prevents Entitlements and/or Application Instance Accounts From Being Included in the Certification Task

Certifications defined with the "All Application Instances" option will not pick up the Application Instances or related Entitlements for a few different scenarios:
SCENARIO 1: In case there is some error reported for an unrelated Application Instance - such as the unrelated Application Instance throws an error due to missing Certification related requirements (ie...item does not have the AccountName property mapped). The error could be like:
SCENARIO 2: When no errors are seen while adding the Application Instance accounts to the Certification task - but in the case where the selected Entitlement or it's related Application Instance are not requestable.
The issue occurs after applying the January 2017 or April 2017 OIM Bundle Patches.
