Symptoms

On : 12.1.3 version SOA Suite, and later

From the EM console, soa-infra data is not accessible after restarting all managed servers.

When clicking on "soa-infra" in the EM console, the flow instances tab shows:

"Error fetching flow instances. Failed with access denied ("oracle.fabric.permission.InstancePermission" "*" "monitor")

Checking soa-infra > Security > Application Roles,  we can see that the 'soa-infra' application stripe is missing, and no roles are shown.

Using WLST, this is confirmed with the following command:

wls:/soa12c_domain/serverConfig> listAppStripes()

Service_Bus_Console
wsm-pm

It should look like this:

soa-infra <<<----

Changes

 The 'SOAINFRA' application was redeployed from the Admin Server, which removed the associated application stripe, and corrupted the OPSS Schema data.
