FRM-40010: Cannot Read Form with 12c Release 12.2.1.3.0 (Doc ID 2305713.1)

Symptoms

Attempting to run a 12.2.1.0.0, 12.2.1.1.0, or 12.2.1.2.0 Form in a 12.2.1.3.0 installation environment and receiving:  FRM-40010: Cannot read form <form name>.

The Forms executables (.fmx, .plx, .mnx) are in the FORMS_PATH.

The solutions mentioned in the following notes do not apply:

Changes

 Installed FMW Forms and Reports 12.2.1.3.0.
