Session State Violation in APEX 5.x When Accessing Cascading LOV's (Doc ID 2310738.1)

Symptoms

When attempting to create a new cascading LOV in APEX 5.x the following error occurs:

ERROR
-----------------------
"Session state protection violation: This may be caused by manual alteration of a URL containing a checksum or by using a link with an incorrect or missing checksum. If you are unsure what caused this error, please contact the application administrator for assistance."


Steps to reproduce:

The issue can be reproduced at will with the following steps:
1. Create a new cascading LOV
2.Try to access it
3. The session state protection error is seen.
