OVD 11g: ODSM GUI Does Not Render Properly After Changing Ping Protocol to LDAP (Doc ID 2311256.1)

Symptoms

After changing the Ping Protocol to LDAP from TCP in Oracle Directory Services Manager (ODSM), on subsequent page renders list boxes and radio buttons on the GENERAL tab for this adapter are rendered as simple text fields.  This affects the SSL Authentication Mode, Failover Mode, Use DNS for Auto Discovery, and other fields using list box pull downs or radio button selections.
